Overview
Breast revision is a secondary cosmetic surgery designed to correct or enhance the results of your previous breast augmentation. Over time, changes or complications such as implant rupture, capsular contracture (hardening of scar tissue), implant shifting, or dissatisfaction with implant size or shape may lead you to seek a revision procedure. This may involve replacing old implants with new ones, adjusting the implant pocket to correct implant malposition, or removing implants altogether if you choose. In some cases, a breast lift (mastopexy) is performed simultaneously to restore youthful contour and firmness. Fat transfer to the breasts can also enhance the results of your breast revision.
During your breast revision consultation, Dr. Khoobehi uses the VECTRA® 3D imaging system to capture detailed 3D images of your chest to simulate potential outcomes, giving you a chance to virtually “try on” different options. This advanced technology enables you and Dr. Khoobehi to visualize different implant sizes, shapes, and placements, helping you make informed decisions and ensure your results align with your goals.
Breast revision is a highly individualized surgery specifically tailored to your needs and desired results, whether the goal is to correct complications or improve aesthetics. When performed by an experienced plastic surgeon like Dr. Khoobehi, breast revision surgery can restore your confidence and deliver more natural-looking, balanced, and beautiful results that align with your current goals and body.
No matter whether you choose to replace or remove your breast implants, Dr. Khoobehi recommends pairing breast revision surgery with fat transfer to the breast to refine your results and give your breasts a more natural look and feel.
Reasons For Revision Surgery
Breast revision corrects circumstances where a previous breast augmentation procedure has resulted in an unsatisfactory outcome. The surgery focuses on removal, replacement, or making a switch from saline to silicone-based implants. Some of the most common reasons why you may choose to undergo revision surgery include:
Breast Implant Removal
Individuals who undergo breast augmentation do so to increase their breast size, primarily for aesthetic reasons. However, after breast augmentation, some women may be dissatisfied with their results and opt to revert to their previous breast size. Others may want to remove their implants because of health concerns or other personal reasons. Breast revision surgery is a procedure in which the implant is removed and the skin over the breast is tightened to ensure the breast sits in a natural position. Combining breast implant removal with fat transfer is recommended to enhance the overall look and feel of your natural breasts.
Ruptured Implant
Despite the guarantee that implants are safe and leaks are rare, there are cases in which the silicone implant shell breaks down, leading to a leak or rupture. This is known as implant failure. If the implant is filled with saline, the solution will be absorbed by your body, and you will see a noticeable deflation of your breast. In the case of silicone implants, the thick, cohesive gel typically stays within the breast pocket, and the only way to verify a possible rupture is through MRI or ultrasound imaging, as the implant will retain its shape.
Capsular Contracture
When you get breast augmentation, your body creates a capsule of scar tissue around each implant. In most cases, this scar tissue remains soft and pliable, but in some cases, it hardens and constricts around your breast, a complication known as capsular contracture. As the scar tissue hardens, it tightens around the implant, which causes your breasts to feel hard and uncomfortable and look misshapen. Capsular contracture can only be corrected by removing the implant and performing a capsulectomy to surgically remove the scar tissue. You may then choose to replace your implants or leave them out altogether.
Breast Rippling
For patients who have little body fat or natural breast tissue, implant rippling can be a concern. For thin patients with low body fat or those without enough natural breast tissue to adequately cover the implant, the edges may be visible, and noticeable rippling can sometimes be seen beneath or along the sides of the breasts. While implant rippling is not really considered a complication, the unnatural wrinkling may cause discomfort and an unsightly appearance. Rippling is most common with saline implants. Implant rippling can often be mitigated by fat transfer to the breast, as the fat will smooth the transition between the breast and the implant.
Double Bubble Implant Deformity
When an implant moves out of the intended breast pocket, it may create a double-breast appearance, with a noticeable crease. Implants that are not properly placed may drop and extend past your breast crease (the area where your breast meets your chest wall) or rise, forming an odd volume on top of your breasts. Implant displacement may occur for several reasons, including improper surgical technique or capsular contracture. Revision surgery is the only way to correct a double bubble deformity.
Breast Size Adjustment
Breast implants vary widely when it comes to size, texture, shape, and base width. Some patients may be unhappy with how their breasts look after augmentation and may consider revision surgery to improve their appearance. Revisions may include a size increase or decrease, or a switch from a rounded implant to a more natural-looking teardrop-shaped implant. To help you decide on the appearance you desire, Dr. Khoobehi will use the VECTRA® 3D imaging system during your consultation. He will modify the images using different implant types and sizes based on your tissue type and chest width. He will then use those images to help you decide which breast implant is right for your particular body type and desired results.
Surgical Technique
Your revision surgery will be tailored to address your specific aesthetic concern or implant complication. With this treatment, the augmented breast is corrected in conjunction with scar tissue removal (if applicable), a breast lift if there is sagging, or fat transfer to improve appearance. There are rare instances when a strattice (a tissue-matrix device) is needed as an internal underwire when additional implant support is required. Ultimately, the goal of breast revision is to address the issue causing your discomfort or dissatisfaction.
Dr. Khoobehi will remove the implant through the same incision used in your previous surgery, avoiding new incisions and minimizing scarring. He will then make the necessary adjustments to your breasts, whether that includes replacing your implants, permanently removing them, releasing the scar tissue causing capsular contracture, or correcting implant displacement.
If you and Dr. Khoobehi determine that replacing your implant is the best solution to achieve your goals, he usually recommends submuscular placement, which is the placement of the implant beneath the pectoral muscle. This usually achieves the most natural-looking results.
Once the breast revision is completed according to your surgical plan, Dr. Khoobehi will close the incisions, and you will be moved to recovery.
Fat Transfer to the Breasts During Breast Revision
You may choose to combine your implant removal or replacement with fat transfer to the breasts. This addition provides a smooth transition between the existing breast tissue and the implant, giving your breasts a more natural feel. Dr. Khoobehi calls this technique “cleavage therapy.” The procedure begins with gentle liposuction to harvest fat from areas of your body with excess volume. The collected fat is then carefully purified to isolate the healthiest, most viable cells and placed in a small syringe. Using a precise micro-injection technique, Dr. Khoobehi layers these fat droplets throughout your breasts to create smooth, symmetrical, natural-looking fullness with lasting results.
If you are interested in fat transfer, Dr. Khoobehi will determine whether you are a good candidate during your breast revision consultation. Ideal candidates for fat transfer to the breasts have sufficient excess body fat to harvest for the procedure and sufficient natural breast tissue to support the fat graft. You will need to undergo a mammography prior to surgery and follow up with a second one a year after your surgery. You should also have realistic expectations about what results can be achieved with fat transfer to the breasts.
During/After Surgery
Breast revision surgery is performed with general anesthesia in an accredited outpatient surgical facility. You must have a responsible adult drive you home after surgery and stay with you to provide support for approximately 48 hours.
Recovery after your breast revision surgery will vary depending on the complexity of the procedure, but in general, it will follow a similar timeline as your original augmentation. You will initially experience mild swelling, bruising, and discomfort for the first few days, which can be managed with prescription or over-the-counter pain medication and rest. A surgical bra helps minimize swelling and support your breasts as you heal. You will be able to resume light activity about two weeks after surgery, though strenuous exercise should be avoided for four to six weeks. As swelling subsides, your breasts will gradually settle into their new shape, eventually revealing smoother, more refined contours and improved overall symmetry.
Expected Cost
Breast revision surgery starts at $8,000 but varies based on several factors. The complexity of the procedure, the type of implants you choose, and whether you add additional cosmetic procedures (like a breast lift and/ or fat transfer) will all figure into the final price of your breast revision. When implant replacement is necessary due to a manufacturer’s defect, warranties may cover a portion of the surgical costs.
Dr. Khoobehi is a board-certified plastic surgeon known for his expertise and personalized approach in performing breast revision procedures. He will work closely with you to develop a treatment plan tailored to your specific needs and goals, ensuring that you achieve the best possible outcome.
FAQs About Breast Revision
Model
How soon can I have a revision?
Dr. Khoobehi recommends waiting at least six months to a year after your initial breast augmentation before considering a revision. This allows your breasts to fully heal, for swelling to completely resolve, and for your breasts to settle into their new shape. In many cases, what may initially seem like asymmetry or implant malposition early on in your recovery may improve as healing progresses. Performing revision surgery too soon can interfere with proper tissue healing and negatively affect your final results. However, if you’re experiencing complications such as implant rupture or severe capsular contracture, you may need earlier intervention.
How long do the results last?
The results from your breast revision surgery are designed to be long-lasting, especially when performed by an experienced surgeon (like Dr. Khoobehi) using modern techniques and high-quality implants. If you replaced your implants during revision surgery, you can expect them to last 10 to 15 years or longer. Fat transfer to the breasts is also long-lasting; however, your results can be affected by weight fluctuations, aging, and hormone changes.
What about scarring?
Whenever possible, Dr. Khoobehi will use your original breast augmentation incision sites for your breast revision procedure to minimize new scars. If additional incisions are needed (for revision or for liposuction used in fat transfer to the breasts), they’re placed discreetly. With proper care, any scars from your breast revision and fat transfer to the breasts typically fade and become barely noticeable over time.
