Combining Breast revision, Breast Implants &/or Fat transfer can provide outstanding results!

However, some women find themselves feeling dissatisfied with the results of their surgery because of persistent pain, or aesthetics that fall short of their goals. If you are not satisfied with the results of an augmentation procedure, New Orleans aesthetic surgeon, Dr. Kamran Khoobehi can perform breast implant revision surgery to help you achieve the comfort and appearance you deserve. Dr. Khoobehi has performed many successful breast revision procedures for cases both complex and common, using both traditional and advanced techniques. Dr. Khoobehi performs a 3D image called Vectra which provides the capability of altering the images which gives a visual of the expected desired look. Dr. Khoobehi recommends pairing breast revision surgery with Fat transfer to the breast to achieve a natural look and feel.

Reasons For Revision Surgery

Breast revision corrects circumstances in regard to breast implants, breast asymmetry or other surgeries that have resulted in an unsatisfactory outcome. The surgery focuses on removal, replacement, or making a switch from saline to silicone-based implants. Dr, Khoobehi recommends Fat transfer during either complete removal of implants or making a switch.  The Fat transfer to the breast gives a natural transition between the breast tissue and the implant, ultimately providing a natural, smooth look.  Listed below are the most common cases where a patient may be a candidate for this treatment.

Individuals who wish to undergo breast augmentation do so to increase breast size primarily for aesthetics. However, after breast augmentation, some women may be dissatisfied with their results and opt to revert back to their normal breast size. Others may want to remove their implants because of health concerns or other personal reasons. Breast revision surgery is a process where the implant is removed and the skin over the breast is tightened to make sure the breast sits in a natural position and looks normal, but combining the fat transfer will enhance the overall look and feel of your natural breasts.

Despite the guarantee that implants are safe and leakages are rare, there are cases when a shell breaks down. If it is filled with saline, the solution will be absorbed by the body, and there will be a noticeable deflation of the breast. In the case of silicone implants, the gel will settle where it is originally placed and the only way to verify a possible rupture is with an MRI because the gel doesn’t deflate.

The tissues surrounding the implant may develop scars over time, which creates a capsular contracture or hardening of the breast. The scar tissue tightens around the implant, which causes the implant to be displaced. The patient may feel hardening of the breast, which can be discomforting. This is corrected by a capsulectomy in which the scar tissue is surgically removed. Based on statistics, there are more occurrences of contracture with saline implants compared to silicone-based ones.

A noticeable rippling can sometimes be seen underneath or on the side of the breast. This is generally more common among patients who have very low tissue volume and who are thin. Combining fat transfer to breast revision surgery aids those who have low tissue volume and whom are thin. Although not really a complication, the unnatural wrinkling may cause discomfort and an unsightly appearance. Rippling is most common with saline implants.

When an implant moves out of the intended pocket, it may cause the appearance of a double breast where a significant crease is noticeable. Implants that are not properly placed may drop and extend past the breast’s crease or rise, forming an odd volume on top of the breasts. Sometimes implants are displaced by severe capsular contracture or a tightening scar tissue. Patients who experience this normal opt to have the revision surgery as this condition is usually noticeable when wearing tight-fitting clothes.

Breast implants vary in size, texture, shape and base. Some patients may be unhappy with how the breast looks and may consider a revision surgery to improve the appearance according to preference. It may be another size increase or a switch from a rounded implant to a more natural looking teardrop shape. To help you decide what appearance you desire, Dr. Khoobehi will utilize the 3D Vectra at your consultation.  Dr. Khoobehi will modify the images with different  implants and sizes based on your tissue type and chest width which he will present with you.  The 3D Vectra is a image that helps you see your new potential self!

Expected Cost

A breast revision surgery starts at $8,000 depending on the surgical case. The cost will vary depending on if there are other additional procedures, breasts implant replacement, breast lift, and/ or fat transfer. In cases where implants are a manufacturer’s defect, warranties may cover a portion of the surgical cost. Dr. Khoobehi is known for his expertise and personalized approach in performing Breast Revision. He works closely with each patient to develop a treatment plan that is tailored to their specific needs and goals, ensuring that they achieve the best possible outcome.

Surgical Technique

Revision surgery is tailored according to the aesthetic issue the patient is addressing. With this treatment, a major correction is done to the augmented breast in conjunction to scar tissue removal, if applicable, or a breast lift if there is sagging. There are rare instances when a strattice (a tissue matrix device) will be needed to act as an internal underwire if additional support is required. Ultimately, the correction aims to fix the issue that’s the source of the patient’s dissatisfaction.

If you and Dr. Khoobehi determine that replacing your implant is the best solution to achieve your goals, he usually recommends submuscular placement, or the placement of the implant beneath the pectoral muscle. This usually achieves the most natural-looking results. In some cases, implant removal or replacement can be combined with fat transfer.  This combination will provide a smooth transition from the breasts tissue and implant while giving the breast a natural feel. Dr. Khoobehi will remove the implant thru the same incision from previous surgery this technique is amazing, it prevents a new incision and/or scar. If you are interested in fat transfer, below are a few thing to talk to Dr. Khoobehi at your consultation:

  • Have enough body fat for the procedure
  • Undergo a mammography with positive results
  • Have enough breast tissue to facilitate the fat graft
  • Follow up with another mammography a year after surgery
  • Have realistic expectations

During/After Surgery

Breast revision surgery is performed with general anesthesia in an accredited surgical facility, Outpatient Surgical Center. It is recommended to have to have a ride to and from the Outpatient Surgical Center and who can provide support for approximately 48 hours. Patients can resume to their normal routine around the second week. It can take a few more months for the effects of the revision to become noticeable and the breast to develop a natural feel and look.
